Good to know...

Business name and address and phone are REQUIRED fields. Phone and email contact information is optional but HIGHLY RECOMMENDED. 

Name entries such as "Jane & John" are acceptable if that is how you would like it to appear. HOWEVER, data submitted in this format will ONLY be alphabetized and listed by the first name typed in the name field, in this case "Jane." 

For couples with different last names, you must submit your information separately.
