You may include home phone numbers, work numbers, mobile numbers and email addresses for each person in your household. This information is invaluable in cases of emergencies. If you are not a resident, please use the general form for Associate and Honorary Lifetime Members: Non-resident Form.

Good to know...

Name and address are REQUIRED fields. Phone and email contact information is optional but HIGHLY RECOMMENDED. 

Name entries such as "Jane & John" are acceptable if that is how you would like it to appear. HOWEVER, data submitted in this format will ONLY be alphabetized and listed by the first name typed in the name field, in this case "Jane." 

For couples with different last names, you must submit your information separately.

If you would like multiple email addresses listed, please submit the completed form for each.

** Property management companies should use the "Last Name" field for their company name. Please leave the first name blank or enter a contact persons name.

Printed copies of the directory are only shared with SPHO members (Oakwood residents, Oakwood property owners, and Honorary & Associate SPHO Members.
